Expert Patient Programme

To assist your clinic in reporting back on the impact of the Expert Patient Programme, we have drawn up day sheets for expert patients which you can download from this link, as well as a report back format for expert patients and their supervisors.
If your clinic would like to apply to join the expert patient programme, please read through the information contained in this link and submit your application to info@teampata.org.
6 month supervisor reports are due on the 30th June 2008.
PATA will use three different sources for evaluating the impact of the Expert Patient Programme, namely:

  • Expert Patient supervisor reports (in June and November) - these will be based on the day sheets that expert patients will need to complete. Clinics will be contacted individually with these.
  • Expert Patient narratives 'stories' (in April and October) - Specific questions will be emailed to expert patient supervisors who will need to give these to their expert patients and fax them back once completed.
  • Telephonic interviews with EPs and Supervisors (in April, May and October)
